Application and Registration Documents

 

If you wish to enroll in the Urban Youth Worker Certificate please fill out the application and return it either by fax, email or mail.  You may take classes at the certificate level before applying, but the Urban Youth Worker Certificate will not be granted to you until you are accepted in the program.  You may also take the class as an audit in which you will not receive the Urban Youth Worker Certificate.  To register please download the appropriate form and indicate on the form whether you are taking it on a certificate or audit level.  Please do not be confused by the pricing on the registration form.  The price for the certificate level is $80 per course and $50 per course for an audit.
